Benefits

How can I order a new medical insurance card?
You can order a new insurance card by going to www.bcbstx.com and register to use the website. Under Quick Links you can order a new card and print out a temporary card.
 

What else can I do on www.bcbstx.com(opens external link in new window)?
You can track your medical claims, view coverage information, chat live with a BlueCross BlueShield of Texas customer service representative, download medical claim forms, participate in Blue Points, access Personal Health Manager, price prescriptions and track prescriptions that have been mail ordered.

I need to file a flexible spending claim, where can I find the form?
The flexible spending claim form and the instructions to file the claim can be found on the Total Administrative Services website.

Is my baby covered on my health plan immediately upon birth?
No, the baby is not automatically covered on the mother’s policy. No expenses for the baby will be covered unless an enrollment form to add the baby to our health plan is received within 31 days of the birth of the child.

When can I add or remove someone from my Benefits plans?
You must have a “qualifying event” such as the birth of a child, loss of job, marriage, divorce, new benefits as the result of a job change. Written documentation of the “qualifying event” and enrollment forms must be submitted to the Benefits/Payroll office within 31 days of the “qualifying event” date.

How can I find providers and order a new card for Dental and Vision?
The website for Ameritas is www.ameritasgroup.com. This website can be used to access benefit information for you and your covered dependents. View a benefit summary, certificate of coverage, pending and paid claims, pretreatment estimates and the status of plan maximums and deductibles. You may also access valuable resource information, including forms, a glossary of terms, Frequently Asked Questions (FAQs) and instructions on how to nominate a dentist or specialist. Ameritas does not issue ID cards. You can find providers by going to the website www.ameritasgroup.com. Your provider can confirm your benefits by calling 800-487-5553.

I am going to be out on medical leave and I have disability insurance, will I need to file a claim?
Yes, there is claim paperwork that both you and your doctor will need to complete and return to the Benefits Office. Please contact a member of the Business office staff and they will send you the claim forms and the instructions for completing the forms.

 



Accounts Payable and Reimbursements

Will my reimbursement for travel and supplies be included on my Payroll check?
No, you will receive a separate check from the Accounts Payable Department.
 
How soon after you receive my request for reimbursement will I receive my check?
Reimbursements are processed and mailed weekly. In most cases, reimbursements received by Tuesday of each week will be processed and be available for pick-up or mailed on Friday.
 
What are the maximum amounts for reimbursements?
Mileage: .50 cents per mile
Meals: $10 Breakfast, $12 Lunch, $20 Dinner

Where can I find the official state mileage between two cities?
Official state mileage can be found here. The electronic mileage guide is adopted under TEX. GOV'T CODE ANN. §660.043(c) (Vernon Supp. 2004). It is intended as a guide for employees to use when traveling by automobile. If a location is not listed in the mileage guide or an employee chooses to claim actual miles driven, the employee may document point-to-point mileage using an odometer reading. Please use the district adopted mileage rate at the time of travel when accessing the guide.

How soon after you receive my requisition for payment to a vendor will the check be prepared?
Check payments are processed and mailed weekly. In most cases, requisitions indicated as received and ready for payment by Tuesday of each week will be processed and be available for pick-up or mailed on Friday.
